Insurance companies require preauthorization prior to your examination.
Preauthorization is not a guarantee of payment and you may be responsible
for any claims that are denied, applied to your deductible or any
coinsurance amount.
Southwest Vision does not share any personal health information
with a third party except as required by your insurance company.
You may have a "well vision" plan
providing coverage of routine eye examinations and a medical plan
which may cover certain exam procedures relative to medical findings
or diagnosis. Our staff will explain your covered services at the
time of service.
